The award-winning Survivorship Care Plan Builder is closing a critical gap in cancer care. This tool – used by thousands of oncology professionals today – is the flagship resource of a collaboration of organizations with the common goal of improving survivorship care.

Project

The Survivorship Care Plan Builder is an easy-to-use software program that NearSpace designed, built and maintains on behalf of Journey Forward. The Survivorship Care Plan Builder works by prompting the user to create a unique Survivorship Care Plan for each patient based on best practice guidelines. These Care Plans can then be printed out, emailed, exported to a different file format or stored electronically. A number of built in utilities – such as a calendar for scheduling follow-up care, a regimen library, medical calculators and pick-and-choose forms – make the task quick and easy. Survivorship Care Plans may also include documents from the product’s Library on topics like Symptoms to Watch for Following Treatment or Managing Fatigue.
